Our service area includes the counties of Albany, Bronx, Columbia, Dutchess, Greene, Herkimer, Kings, Montgomery, Nassau*, New York, Oneida, Onondaga, Orange*, Oswego, Queens, Rensselaer, Richmond, Rockland*, Schenectady, Suffolk*, Ulster and Westchester* Counties in New York. You must live in one of these places to join Fidelis Medicare Advantage plans.

 

* Fidelis Medicare Advantage (HMO-POS), Fidelis Medicare Advantage Part B Reduction (HMO-POS) and Fidelis Dual Advantage (HMO) are not available in these counties.

 

Fidelis Care is a Medicare Advantage organization with a Medicare contract. Our contract with the Centers for Medicare & Medicaid Services (CMS) is reviewed and renewed on an annual basis. The availability of coverage beyond the end of the year is not guaranteed.
