Stay Covered With Fidelis Care: Recertify Your Coverage Every Year
Fidelis Care wants to make sure you always have the care you need to take good care of your health. Each year, you must recertify, or renew, your coverage. Failure to do so could result in loss of coverage.
You will receive a recertification packet in the mail a few months before your coverage is set to end. To keep your coverage, fill out the forms in the packet and return them to the address listed by the date given.
