Remember to Recertify Your Coverage
As a Fidelis Care member, you must recertify or renew your health insurance coverage every 12 months. When it is time to renew your coverage, you will receive a recertification package in the mail that you must fill out and return to Fidelis Care, your local Department of Social Services, or the Human Resources Administration (HRA). Where you send the completed forms will depend upon where you live and what coverage you have.
If you do not complete and return the forms by the date listed, your health insurance coverage with Fidelis Care will end. We hope you will take the time to renew your coverage, so you can continue getting the care you need to stay healthy.
