Fidelis Care is pleased to announce its newest program, Fidelis Medicaid Advantage Plus (HMO), a managed long-term care program for people 18 years of age or older with Medicare and Medicaid coverage who have a chronic illness or disability. Members must be in need of long-term care services and may need care typically given in a nursing home.

Some of the program’s covered services include: primary care and specialty care physician office visits, home health care, medical social services, adult day health care, social day care, home delivered or group meals, ambulance service, outpatient rehabilitation, preventive screenings, and Medicare Part D prescription drug benefits.
