Fidelis Care believes that everyone deserves access to quality, affordable health coverage. Working closely with doctors, hospitals, and healthcare professionals in your neighborhood and across New York City, we always want to ensure that you and your family receive the best possible service and care. As part of Fidelis Care, CenterCare members now have access to a statewide network of 39,000 providers!

CenterCare and Fidelis Care offer three different programs to meet your needs: Child Health Plus for children up to the age of 19; Family Health Plus for adults ages 19-64; and Medicaid managed care for people of all ages who meet income guidelines.

It's important for you to understand your health benefits and how you can participate in the special programs that CenterCare and Fidelis Care offer. Our Member Services Representatives speak English, Spanish, Chinese, Creole and many other languages. We are always here to help and to answer any questions you may have.

Special Announcements for Providers

CenterCare Reduces Number of Services that Require Pre-Authorization

CenterCare will no longer require pre-authorization for chemotherapy administration, radiation therapy, dialysis, and most ambulatory surgery procedures with dates of service beginning April 1, 2008 that occur in the office, hospital outpatient or free standing surgical center.

Child Health Plus

Quality, affordable care for New York's kids up to the age of 19. As part of New York State's Child Health Plus program, kids are covered for regular checkups, immunizations, routine dental and eye care, prescriptions, hospital and emergency care and more -- all the care they need to keep them happy and healthy.

Family Health Plus

Complete health coverage for adults, ages 19-64. CenterCare Family Health Plus is part of New York State's Family Health Plus program for uninsured adults who are residents of New York State and are not eligible for Medicaid.

Medicaid

Health coverage you can count on for you and your family -- at no cost. CenterCare Medicaid is an easy-to-use health plan that provides personal care from your own doctor, hospital and emergency care, maternity care (ask about our InfantCare and BabyCare programs), dental care (in some areas), eye exams, eye glasses, and more.
